Welcome

On behalf of our team of caregivers and support staff, thank you for considering our healthcare center. We offer a full range of medical services to meet the individual needs of our residents, including short-term rehabilitative services and long-term skilled care. Working with the resident’s physician, our staff – including medical specialists, nurses, therapists, nutritionists, case mangagers, dietitians and social workers – establishes a comprehensive treatment plan intended to restore you or your loved one to the highest practicable potential. Our goal is to provide quality care in a comfortable, compassionate environment, demonstrating on a daily basis our guiding principle of providing Quality Through People.

Oak Hill is staffed with a dedicated and highly skilled staff of licensed nurses and certified nursing assistants, as well as a full range of therapists including Physical, Occupational and Speech Therapists. Our interdisciplinary team also includes a dietitian, an RN Utilization Coordinator to monitor the resident’s care plan, Social Workers to meet the resident’s psychosocial and emotional needs and other support staff and consultants to assist in the development and implementation of the resident’s care plan as required.

Overview

Oak Hill Nursing and Rehabilitation Center accommodates 139 residents. We are located in Pawtucket, Rhode Island, approximately five miles from Providence, the state's capital city. Throughout our 47-year history, Oak Hill has developed a reputation as one of the finest healthcare centers in Rhode Island. Our center consists of three separate units. A 34-bed post-acute unit provides care to the medically complex and those in need of extensive rehabilitative services. A 20-bed Alzheimer’s Special Care Unit provides a unique style of care utilizing habilitative therapy in comfortable, home-like surroundings. The remaining 89 beds provide skilled nursing care for those who need help in caring for themselves. Oak Hill is a 2004 recipient of the American Health Care Association’s Quality Award and we are accredited by The Joint Commission (formerly JCAHO).

Our center is dually certified for Medicare and Medicaid, and participates in many commercial insurance plans.
